How to Increase Follow-Through Between CBT Sessions

Follow-through is rarely about willpower. It’s about how many steps your client has to take between the moment they should do the homework and the moment the form is in front of them.

On paper, the steps are: remember the assignment exists, find the folder, find the right page, find a pen, find a private spot, write legibly, store the page somewhere they’ll remember, bring it back to next session. Each step is a friction point where the homework can drop out.

A digital form on the device they’re already holding cuts the steps to two: open the app, fill in the form. Everything else is gone. The folder, the pen, the private spot, the storage, the carrying-back. None of those exist anymore.

This is the lever for follow-through. Not pep talks. Not accountability questions. Friction reduction.

In my-cbt, the assignment lives in the client portal on your domain. They open the portal on their phone (one tap from their home screen, if they’ve added it), see the assignment with your one-line message at the top, fill in the fields, hit submit. The submission is timestamped and saved in the case file. You see it on your dashboard within seconds.

Set a clear when. The form that says “fill in this week” gets filled in less often than the form that says “fill in the next time you feel anxious before a meeting.” A trigger-based instruction gives the client a clear cue. They open the form when the cue happens, instead of waiting for a vague “this week” that never quite arrives.

Use Kudos. Each completed assignment adds visible points to the client’s total. The reward isn’t the point of therapy, but the dopamine bump for hitting submit reduces the cost of starting next time.

A few weeks of high follow-through changes the rhythm of treatment. You walk into session with a week of real data. The conversation goes straight to what’s there. Your client experiences themselves as someone who does the work, which is its own clinical lever.

Setup takes you under a minute per assignment. The friction goes from the client’s side. The numbers move within the first month.
