Negative judgments you make about yourself

Depression, anxiety, eating disorders, perfectionism, post-trauma shame, and any client whose internal monologue is dominated by self-attack. Pair with the judgments-about-others log if both are happening, since the same client often does both.
The form is a single repeating block, at least 3 rows, with 5 sub-fields per row.
A clinical note worth holding: clients who write a more realistic response that’s still subtly self-critical (“I’m not a complete failure, just mostly a failure”). Push for actual balance. And: clients whose harsh self-judgments are essentially direct quotes from a parent or critic. That’s important context that often shifts the work toward the source of the voice rather than the content.
In my-cbt, the form is in the system templates and ready to send. The personal message field is where you frame the task for this client. Submissions show up in the case file alongside the session notes. The repeating sections expand on screen instead of running out of room in a margin.
