Mindfulness practice log

Clients new to mindfulness, clients who can’t or won’t commit to formal sitting practice, anxious clients who need a low-stakes entry point. Daily practice is the only thing that builds the muscle, and informal practice has a much higher compliance rate than formal meditation in the early weeks.
The form has 2 fields. “Each day this week” is a repeating block with at least 7 rows, each with 4 sub-fields (date/time, short-text, long-text, slider).
A clinical note worth holding: clients whose mood-after entries are consistently “fine” or “the same.” That can mean the practice isn’t yet engaging, or it can mean they’re dissociating during it. Ask what they actually noticed in the activity column. Specific sensory observations (“the bread was warmer than I expected”) are a sign of real attention. Vague responses (“it was okay”) usually mean they went through the motions.
In my-cbt, this is a system template. Assign it from the case file, customise the personal message, and the submission saves with a timestamp when the client submits. The repeating sections expand on screen instead of running out of room in a margin.
