Activities that lift your mood

Depressed clients who’ve withdrawn from things they used to enjoy. The mood-before / mood-after rating exposes the gap between predicted and actual enjoyment, which is usually wider than the client expects. Anhedonia distorts both anticipation and recall, so the data matters.
The form has 3 fields. “Five activities you’ll try this week” is a repeating block with at least 5 rows, each with 1 sub-field (short-text). “Each time you do one of them” is a repeating block with at least 7 rows, each with 4 sub-fields (date/time, short-text, slider, slider).
A clinical note worth holding: clients whose mood-after consistently equals mood-before despite real engagement. That’s clinically meaningful and points toward more severe anhedonia or a missed diagnosis. Also watch for clients who pick five ambitious activities and complete none. Help them shrink the bar.
In my-cbt, the form is in the system templates and ready to send. The personal message field is where you frame the task for this client. Submissions show up in the case file alongside the session notes. The repeating sections expand on screen instead of running out of room in a margin.
