How to Review Thought Records in Session

The default is to go through the thought records one by one, in order, asking “what was happening here, what were you thinking, what did you do.” It works for the first few weeks. After that, the field-by-field review starts to feel like admin, and the data stops producing anything new.

There’s a faster, more productive structure.

Open the case file. Look at the week’s thought records and pick the highest-distress entry and the lowest. Read them next to each other. The conversation becomes about the difference: what was happening at the 9 that wasn’t happening at the 4, what they were thinking, what they did or didn’t do. The contrast tells you what’s driving the variance.

This is what your clinical training implicitly teaches: the data is most informative at its extremes. The middle entries are the ones your client could already manage on their own. Looking at them in session is mostly a check-in disguised as analysis.

If the highest-distress entry is at 9 with the thought “I’m going to lose my job”, and the lowest-distress entry is at 4 with the same thought, the conversation is about what was different in the situation. The thought was the same. The context was different. That contextual difference is what shifted the distress. You’d have missed that finding scanning all five entries in order.
