Social activities you'll try

Isolated clients, depressed clients who’ve withdrawn, anxious clients who avoid social contact. The point is not just selection but commitment to one activity per day. In-person beats online for clinical effect, so encourage the in-person items even when the client is drawn to the safer online ones.
The form has 2 fields. “Social activities you’ll try this week” is a repeating block with at least 5 rows, each with 2 sub-fields (short-text, short-text).
A clinical note worth holding: clients who pick five activities they would never actually do. Help them shrink to one or two genuinely doable ones. Also: clients who pick all solo-feeling activities (a course, a class) and skip the relational ones (dinner with a friend, calling a relative). Both count, but the relational ones do more for loneliness.
