Clients who say they “already know” what helps but don’t do it. The point of the worksheet isn’t education, it’s the three-week tracking. The pattern of which categories they actually engage with versus skip tells you more than the initial planning.

The form has 3 fields. “Stress-reducing activities you’ll commit to” is a repeating block with at least 5 rows, each with 4 sub-fields (short-text, dropdown, short-text, slider).

A clinical note worth holding: clients who pick six items in one category and zero in others. That imbalance is usually meaningful. Workaholics skip self-care, lonely clients skip social activities, depressed clients skip creative and fun.
